RN/Per Visit, Healthy @ Home-N. Meck/Cabarrus, hours vary/as needed (PRN/No Benefits)

Provides nursing services to home care patients according to physician orders, state practice acts and regulatory required under supervision of manager. Maintains clinical and professional competency. Performs evaluation of patients, families and home environment for admission, performs nursing assess and reassess to establish and revise individual plan of care, administers meds, treatments and nursing services, observes and documents signs/symptoms and response to care, reports any reaction to treatments, meds or changes in clients physical or emotional condition, assesses patient and family education and learning needs and provides teaching/training, maintains accurate, and complete documentation, completes and submits visit reports, payroll records, mileage, orders, supply charges and on-call reports according to policy, participates in on-call rotation. Submits weekly schedules, supervises/instructs home health aides, reports deviation from care plan, develops/maintains knowledge of Medicare, Medicaid, third party guidelines, regulatory and reimbursement requirements. Maintains confidentiality, participates in conferences, in-services, committees, process and performance improvement acts. Graduate of accredited school of nursing with NC RN license, minimum of one year nursing experience, experience in home health preferred. Maintains current state issued drivers license. Computer experience helpful. Additional Information Weekend and/or Holiday Rotation may be required.
